Outcome of Venous Supercharged Pedicled Anterolateral Thigh Flap for Reconstruction of Soft Tissue Defect around the Knee Joint

Introduction: Soft tissue defects around the knee are common following motor vehicle accidents, burns especially electric burn, excision of malignancy etc. Reconstruction soft defect needs thin, pliable and tough skin. Recently distally based pedicled anterolateral thigh flap has been used for reconstruction its several advantages like long pedicle, sufficient amount less donor site morbidity. But it tendency towards venous congestion this can be overcome with supercharging. This study was designed to evaluate effectiveness supercharging in reducing joint. Methods: prospective observational carried out at Department Plastic Surgery Burn Unit, Dhaka Medical College Hospital Sheikh Hasina National Institute (SHNIBPS), from August 2017 June 2019. Twenty (20) patients who presented were included according inclusion exclusion criteria. Results: Maximum 5(25%)were day labour wounds created by MVA most cases10(50%).Most anterior aspect joint 8(40%).The dimension wound size ranged 5×3cm upto 7×14cm. Majority 15×13cm 17×14cm. The 6×3.5cm 20×18cm. 18 (90%) flaps had no congestion, but 2(10%) cases it. All survived well, acceptable scar 2 infection occurred. Regarding functional outcome joints, 15 (75%) full range motion 5(25%) 5p -10p restriction flexion joints. Final found excellent 16(80%) satisfactory 4(20%) cases.
